Insurance & Financial Information
Your insurance is a contract between you and your insurance company. We submit claims on our patients’ behalf as a courtesy. We do not know your particular policy’s details. Your coverage is determined through negotiations between your employer and the insurance company. We make every effort to submit a pre-estimate to your insurance company to help determine the probable out-of-pocket cost for upcoming procedures. This amount is collected at the time of service. If your insurance determines that a service is not covered, you are responsible for the complete charge.
- Delta Dental
- Blue Cross/Blue Shield (excluding PPO)
We’re out of network but will submit to:
- HP Pediatric Dental
- United Health Care
Patients who don’t have dental insurance must pay in full at the time of service.
We send statements monthly when a balance is due on your account. Payment is due within 30 days. Accounts may be referred to our collection agency in cases where payment isn’t received after the third statement.
Our relationship is with the parent who usually accompanies children to their appointments. We are not privy to the details of court orders regarding child custody or divorce agreements. Noncustodial parents will not be billed without their written consent.
For your convenience, we accept:
- Payment plans are available for orthodontic treatment or extensive restorative dental work
We value your time, and we pride ourselves on scheduling patient appointments so everyone can be seen in a timely fashion. For that reason, we ask that you provide 24 hours’ notice if you’re unable to keep your appointment. This enables us to better serve everyone. A missed appointment fee may be charged if we do not receive appropriate notice.
If you have any questions regarding these policies please do not hesitate to contact our office manager, who will be happy to talk with you.
